Knee osteoarthritis is progressive wearing of articular cartilage — the smooth surface covering bone ends. As cartilage thins, bone rubs on bone causing pain, stiffness, swelling and reduced function. It is the most common joint condition in adults over 50 in India.
Mild to moderate knee arthritis can be effectively managed without surgery using physiotherapy, PRP injections, hyaluronic acid viscosupplementation, activity modification, and weight management. These measures reduce pain, improve function, and can delay or avoid surgery for years.
HTO is a surgical procedure that realigns the mechanical axis of the knee — shifting load away from the damaged compartment to healthy cartilage. It is suitable for younger patients (typically under 60) with isolated medial compartment arthritis, and can delay knee replacement by 10+ years.
Partial (unicompartmental) knee replacement resurfaces only the damaged compartment, preserving healthy bone and ligament — with faster recovery. Knee replacement is generally considered when: arthritis is end-stage (Kellgren-Lawrence grade 3–4 on X-ray), pain significantly impairs quality of life and sleep, conservative treatment over 6 months has failed, and the patient's overall health allows safe surgery.
