What joint registry evidence shows about hip implant survival

Total hip replacement is among the most reliable operations in orthopaedic surgery, and the registry evidence on how long implants last is correspondingly good. National joint registries record every replacement submitted and note when one requires revision, which gives a far more honest picture than any single surgeon’s series could.
That evidence shows the large majority of total hip replacements remain in place and functioning well beyond fifteen years, with a substantial proportion continuing past twenty. Modern bearing surfaces — particularly highly cross-linked polyethylene — have reduced wear considerably compared with the materials used in earlier decades, and the newer data reflects that improvement.
As with the knee, these are population figures rather than a prediction for any individual. Age at operation is the single largest influence: a hip replaced in a patient in their fifties must survive far more walking cycles than one replaced at eighty. Body weight, activity pattern, bone quality and the accuracy of component positioning all contribute.
Cup position deserves particular mention. A socket placed outside the safe range for angle and version wears faster and dislocates more readily. This is the specific problem robotic assistance is designed to address, and it is why component positioning is planned rather than judged by eye.

Age alone rarely decides it. The concern in younger patients is that the implant must last longer under greater demand, making later revision more likely. Where the hip is destroyed by arthritis or avascular necrosis and pain is disabling, replacement may still be entirely appropriate — with the expectation that revision could be needed in later life.
Low-impact activity such as walking, cycling, swimming and golf is encouraged. Running, jumping and contact sport place repetitive high load through the bearing and are generally discouraged. Deep flexion combined with rotation carries a dislocation risk in the early months; specific precautions depend on the surgical approach used.
Robotic assistance improves the accuracy of cup positioning and leg-length restoration. Since malposition is a known contributor to wear and dislocation, better positioning is expected to help implant longevity — but long-term registry data comparing robotic and conventional hips is still maturing, and it would be premature to promise a longer-lasting hip on that basis alone.
It can be revised: the worn components are removed and replaced. Revision hip surgery is more complex than the original operation, particularly where bone stock has been lost, and outcomes are generally less predictable. This is why the first replacement is planned with the long term in view.
